Complex Presentation

If one or more of the following are present, consider the patient’s condition to be complex:

• Suicidal/homicidal risk 
• Significant and prolonged dysfunction(e.g. WSAS >20)
  • Work and Social Adjustment Scale (WSAS) 
• Possible bipolar disorder
  • Mood Disorder Questionnaire (MDQ) 
• Substance abuse
  • AUDIT-C 
  • DAST-10
• Psychosis
• Personality disorder
  • Borderline personality screener (seven or more positive answers is a positive screen)
• ADHD
  • ADHD screener
  • ADHD scoring instructions
• Elderly
• Is your patient pregnant or wants to become pregnant?
• Diagnostic uncertainty
• Refractory to treatment
